Website use and chiropractic services

These terms address use of the Woodlands Family Chiropractic website and optional patient communications offered by Keyes to Health PLLC, doing business as Woodlands Family Chiropractic, in Texas. They do not replace your treatment consent, financial agreement, privacy rights, or other separately signed patient documents.

Website material is general information, not a diagnosis or individualized medical advice. Visiting this site, using chat, or submitting a scheduling request does not by itself establish a doctor–patient relationship. Treatment decisions require an appropriate clinical evaluation. Results vary; testimonials and descriptions of services do not guarantee outcomes.

For a medical emergency, call 911. Website forms, email, and texts are not continuously monitored and should not be used for urgent symptoms or emergencies.

Appointments, intake, and patient agreements

An appointment request is not confirmed until acknowledged by our office or scheduling system. Provide accurate contact information and let us know of changes. Contact our office directly to cancel or reschedule; opting out of reminders does not cancel a visit.

IntakeQ may be used to collect intake information, signatures, and patient agreements. TrackStat may support patient coordination and reminders. External scheduling is available through our linked Patient Health Centers service. Review each form before signing; these website terms are not consent to treatment, a HIPAA authorization, or agreement to undisclosed charges.

Fees, insurance participation, financial responsibility, and any cancellation or missed-visit charges will be addressed in the practice’s separately provided financial policies. Website information does not guarantee insurance coverage or benefits. A parent or legally authorized representative must provide any consent required for a minor, subject to applicable Texas law.

Optional text-message program

When you separately opt in, Woodlands Family Chiropractic may send appointment confirmations, reminders, scheduling updates, intake notices, and patient-service messages to your provided mobile number, including through automated technology and providers such as TrackStat or IntakeQ. Message frequency varies. Message and data rates may apply.

• Consent is voluntary and is not a condition of treatment or purchase.
• Reply STOP to unsubscribe, or revoke consent by another reasonable method, including calling or emailing our office. A non-marketing opt-out confirmation may follow.
• Reply HELP for help, or call (936) 447-9484 or email frontdesk@wfcwellness.com.
• Marketing requires separate consent where required; agreement to reminders is not agreement to marketing.
• Notify us when your phone number changes. Delivery can be delayed or fail because of carrier, device, or network conditions; do not rely on a reminder as your only appointment record.
• SMS may be accessible to others and is not a secure channel for detailed health information. Alternative communication arrangements can be requested.

Our Privacy Policy explains messaging data use. Mobile information and consent records are not sold or shared for third-party or affiliate marketing or promotions. Service providers may process them only as needed to support messaging, or as required by law.

Responsible use and ownership

Do not impersonate another person, submit information you lack authority to provide, interfere with the site, attempt unauthorized access, transmit malicious code, or use the site unlawfully. You retain your rights in information you submit; submission permits only the handling needed to respond to your request and other handling permitted by applicable law and our privacy notices.

Our branding and original website material, and material licensed from others, are protected by applicable intellectual-property laws. You may view and print material for personal, noncommercial use, subject to legally permitted uses. Other reproduction requires permission from the appropriate rights holder.

External services and availability

The website links to or embeds third-party scheduling, intake, chat, maps, videos, analytics, and other services. Their separate terms may apply. We do not guarantee their availability or the accuracy of external content. Inclusion of a link does not waive our own duties under privacy or healthcare law.

We aim to keep website information accurate, but content may contain errors or become outdated. Confirm information important to your care, appointments, or financial decisions with our office. Access may be interrupted for maintenance, technical issues, or security reasons.

Texas law and your rights

These website terms are governed by applicable Texas law and controlling federal law. Chiropractic services remain subject to Texas Occupations Code Chapter 201 and applicable Texas Board of Chiropractic Examiners rules. Privacy obligations include applicable HIPAA requirements and Texas medical privacy law.

Nothing in these terms waives patient rights, limits responsibility for professional negligence or other liability that cannot lawfully be limited, requires arbitration, or prevents a complaint to a regulator. If a provision is unenforceable, the remaining provisions continue to apply to the extent permitted by law. Changes will be posted with a revised date, with additional notice or agreement when legally required. Changes do not retroactively alter separately signed patient agreements.
